Day Camp Parent Information
Welcome Camp Parents!
We are getting ready to give your camper's a summer full of fun! These are the steps that need to be completed so that the fun can begin!
Complete fully the following forms:
Camp Health History & Permission Form
Skate Waiver (for all campers attending skate camps)
Inclusion Forms (required for any campers with an IEP/BMP/504) See Parent Handbook page 12 for more information. Returning camper form. New camper form.
Chronic Health Forms (for any campers that require the use of a nebulizer, inhaler, epipen, dibetes testing, or other specific medical care). See page 16 of the Parent handbook for more information. All required forms must be submitted to the camp department at least 1 week prior to your child's start of camp.
As camp gets closer check back for information on Camp Open House, Parent's Night, Camp Family Events and weekly up-dates.
